(1) Conduct a search at the Secretary of State’s site to see if another Corporation is already using this name – “[YOUR LAST NAME] Engineering”. Write brief responses to questions 1(a)-1(e) on paper answering the following questions:

(a) Is this name already being used by another corporation?

(b) If not, what is the number and title of the form to reserve the desired name?

(c) What is the fee to reserve “[YOUR LAST NAME] Engineering”?

(d) For how long can the name be reserved?

(e) Once reserved, what are the limitations on consecutive renewals?

Fully complete the form and print it. Turn it in together with the PDFs of your brief report and Articles of Incorporation.
