HAS 4210CBE Rasmussen Required Forms During Admissions intake Process Memo Health Medical Assignment Help
Competency
Examine the structure of the U.S. legal system and government as it relates to ethics, law, and core principles of public health.
Instructions
You have recently been promoted to Health Services Manager at Three Mountains Regional Hospital, a small hospital located in a mid-size city in the Mid-West. Three Mountains is a general medical and surgical facility with 400 beds. Last year there were approximately 62,000 emergency visits and 15,000 admissions. More than 6,000 outpatient and 10,000 inpatient surgeries were performed.
As a means to build trust between the organization and the patients, you have been tasked with the drafting of a memorandum (memo) to all employees that perform patient intake functions. The focus of this Memo is to highlight the necessary forms that need to be completed during the intake/admissions process and an explanation as to why they are necessary. Often patients are handed a form to fill out, but do not receive a proper explanation as to why it is necessary for them to provide the required information. Without the proper information, patients, more now than ever, are hesitant to provide all of the necessary information. You will need to address the following components in your memo:
- Introduction: Explain why you are sending this memo. Provide an overview of the patient trust issues and why it is important for the employees to be able to discuss each of the forms with the patients during the intake process.
- List of the Required Forms: List and describe each of the required forms at admission.
- Importance of each form to the organization: For each form you list, you will need to add why that form is important to the organization and necessary for the patient to complete.
- Conclusion: In this part, you will wrap up the memo and provide additional resources available to the employees if they have further questions.
- Proper Memo Formatting: Â This a professional document and needs to be structured as a memo and not as an essay. You will still need to supply both in text and full citations for the sources of your information. If you need further assistance with how to structure a memo, click on this link.
NOTE – APA formatting, and proper grammar, punctuation, and form required. APA help is available here.

MN 580 Purdue University Primary Care of Children and Adolescence Discussion Writing Assignment Help
Pediatric myocarditis is the inflammation of the muscle wall of the heart. If myocarditis left untreated may result in chronic cardiomyopathy (rare in children). Cardiomyopathy is the enlarging of the heart due to the heart muscle enlarging and thickening; cardiomyopathy can lead to heart failure. Myocarditis is caused by a viral infection such as adenovirus, mumps, varicella, coxsackievirus A & B, Parvovirus B19, echoviruses, and influenza (Li et al., 2019). Autoimmune disorders may cause myocarditis such as celiac disease, Kawasaki disease, lupus erythematosus, RA, sarcoidosis, scleroderma, ulcerative colitis, giant cell myocarditis, and hypereosinophilic syndrome. Myocarditis can be medication-induced.
Myocarditis can present in young athletes as sudden death. The incidence of dilated cardiomyopathy in children ranges from 0.57 to 0.76 cases per 100,000 children. Myocarditis is associated with dilated cardiomyopathy in 27% to 46% of cases. Dilated cardiomyopathy is the primary indication for childhood heart transplantation (Rodriguez- Gonzalez et al., 2019). Many infants and children with myocarditis present with sudden onset of symptoms such as rapid and labored breathing, wheezing, grunting, low blood pressure, cool extremities, and decreased urine output. Older children may complain of fatigue, fever, vomiting, and muscle aches a few days before developing more severe symptoms such as shortness of breath and exercise intolerance. Advanced symptoms are rapid heart rate, erratic and weakened pulse, pale color, sweating, and dizziness. Differential for myocarditis are sepsis, asthma, recurrent vomiting, dilated cardiomyopathy. Diagnostic tests are Chest x-ray, ECG, Echocardiography, and MRI, CBC, ESR, PCR, BNP, troponin, blood culture, thyroid panel. A chest x-ray will indicate enlargement of the heart but no specific indicators of myocarditis. ECG will show an abnormal heart rhythm. Echocardiography is used in diagnosing and monitoring of myocarditis (Bejigi et al., 2019).
