They are distributed in the brain, spinal cord and gastrointestinal tract. When drugs attach to the sites, they change the cells’ electrical characteristics which is reflected in physiological function such as respiration and changes in mood. Buprenorphine is an antagonist at the kappa opiate receptor. This produces dysphoria (Wesson & Smith, 2010).

Naltrexone is an opioid receptor antagonist (Stahl, 2017). This drug is FDA approved for alcohol dependence, blocking the effects of exogenously administered opioids, and prevention of relapse for opioid dependence (Stahl, 2017). Naltrexone works by blocking mu opioid receptors, which prevents binding of exogenous opioids (Stahl, 2017). This mechanism of action subsequently prevents the pleasurable effects of opioid use (Stahl, 2017). For alcohol dependence treatment, Naltrexone works through modulation opioid systems (Stahl, 2017). This reduces the reinforcing effects of alcohol (Stahl, 2017). Benefits for alcohol addiction include increased abstinence and reduced days of heavy drinking (Stahl, 2017).

Methadone is an opioid agonist and buprenorphine is a partial opioid agonist (Alderks, 2017). One study showed patients with opioid dependence who received buprenorphine had a shorter duration of treatment than those receiving methadone (Hickman et al., 2018). Another study reported the risk for death during the first 4 weeks was lower for buprenorphine when compared with methadone (Hickman et al., 2018). Methadone is administered in federally-approved opioid treatment programs only, while buprenorphine can be prescribed by physicians who receive specialized training (Alderks, 2017). Buprenorphine is classified by DEA as schedule III, and methadone is a schedule II drug (Cicero et al., 2018). Due to buprenorphine being a partial agonist, there is a decreased risk for respiratory depression and fatal overdose (Cicero et al., 2018).
